To sign up for a first time dental appointment, you may call our office at (505) 892-8600 or fill out the appointment request form below. Please be sure to put at least 2 dates that are convenient for your schedule.

We will contact you be either phone or email to confirm your appointment. To make your first visit easier, fill out the following forms at your leisure and bring them with you to your appointment:
